The following laboratories, under the direction of the Department of Pathology, have received accreditation by the United States Department of Health and Human Services, Health Care Financing Administration (CLIA) and the College of American Pathologists (CAP). In addition, the Blood Bank of the Clinical and Anatomic Pathology Laboratories has been accredited, through March 30, 2008, by the American Association of Blood Banks. A summary of laboratory identification numbers with expiration dates is provided below.

Please note that the CLIA numbers only reflect the registration of the specific laboratory and are not to be associated with tests performed by other laboratories. In accordance with the regulations, any test method performed and resulted by a laboratory must have been reported to the United States Department of Health and Human Services and associated with the laboratory CLIA number.
